Adult Scoliosis Treatment: Do I Really Need Surgery? 

Most adults with scoliosis do not require surgery. Treatment typically begins with conservative, non-surgical options such as specialized physical therapy, core strengthening, and anti-inflammatory medications to manage discomfort. Surgery is generally reserved for severe cases where the spinal curve causes debilitating pain, nerve compression, or a significant postural deformity that affects daily mobility. 

What causes scoliosis to develop in adults? 

Adult scoliosis generally falls into two categories. First, “Idiopathic” scoliosis that began in childhood can progress slowly over decades. Second, “Degenerative” (de novo) scoliosis can develop later in life due to the natural wear and tear of spinal discs and joints. This often occurs in the lumbar (lower) spine and can lead to symptoms like leg numbness or difficulty standing upright for long periods. 

When is surgery necessary for scoliosis? 

Dr. Khurana generally recommends a conservative-first approach. However, surgical intervention may be the best path forward if: 

  • The spinal curve exceeds 50 degrees and continues to worsen. 
  • Chronic back or leg pain does not respond to physical therapy or spinal injections. 
  • The deformity is impacting lung function or causing severe physical imbalance.
